Integrated care services
Salford City Council and Salford Primary Care Trust are bringing together district nursing and social services to provide adults and older people with local, integrated care teams.
Your integrated care team will be linked to your GP - so, if your GP is in Walkden, you'll get nursing and social services from the Walkden Integrated Care Team.
What are the benefits?
Many people need both social care support and district nursing. The Integrated Care Teams means that the staff can offer you a better quality service, with easier access to both services if you need it.
Independent research has shown that integrated care teams mean less duplication of work, increased number of assessments, improvements in how quickly we responded to requests and improvements in how quickly people received services.
