Help leaving hospital
Before you leave hospital, social workers and health professionals (nurses, therapists and others) will agree a plan with you about how you will manage when you are discharged.
Social workers visit all the hospital wards and are also available on some clinics. To see a social worker, you should either ask the ward or clinic nurse for a referral.
We may suggest an intermediate care service, which is short-term support to help you get back on your feet.
If you need longer-term support, then we can arrange this with you - we'll carry out an assessment first, to work out the best ways to support you.

Age concern offer an aftercare service
This service provides an aftercare service to Salford residents over 60 years who have attended Accident and Emergency departments or emergency assessment wards.
The service provides:
- emotional support for both carers and patients and carers both in hospital and once the patient has been discharged home
- identify any issues the patient may face once they have returned home from hospital
- we will discuss with you what support you have at home, and we can visit you after discharge to check you are managing
- At this time we complete a full home safety assessment, which includes a falls risk assessment
- we complete referrals to Community Occupational therapy and other services to help keep you independent at home and aid your recovery if you are having difficulties once you have gone home we will refer you into the appropriate services and will support you until those services are put into place
- we can also help with basic shopping, prescription collection, and meal preparation.

Repeat prescriptions
Many pharmacies offer a delivery service of medicines and repeat prescriptions. Contact your local pharmacy or GP for more information.
