Single assessment process

This is a photo of two older people

Single assessment process means that people who need a range of social and health care services will not need to be seen by different professionals, all asking similar questions. Instead, one or two professionals will be able to carry out an assessment of someone's needs and share the information with professionals from other services.
This will improve the quality of service that people receive and will be less stressful for those people.
