Carers emergency scheme

A new carers emergency scheme allows Carers to fill in a simple form with details about the person they care for and space to nominate up to three people who can provide care in an emergency.
Once you have registered, you 'll be given a card with your own PIN number so that even if you can't tell people about the person you care for, help is only a phone call away.
You'll register initially with PRT Salford Carers Centre, which will pass your details on to Care on Call, the city council's mobile warden service and to Salford, Stockport and Trafford Crossroads.
In the event of an emergency, a phone call to the 24 hour emergency line, will let people know that you can't provide care. Your nominated carers will be contacted and asked to provide care during the immediate crisis. If they can't help, then Crossroads can provide short-term emergency care free of charge.
If you'd like to register, you can print off the form below, fill it in and either return it to the Salford Carers Centre or give it to your social worker. for more information about the scheme, contact Salford Carers Centre by phoning 0161 833 0217 or by emailing carers.centre@salford.gov.uk.
