Community Support
Bolton, Salford and Trafford Mental Health Trust have support workers based in four Community Mental Health Teams. We cover the whole city and provide care and support on both a short term and long term basis. If you have severe and enduring mental health needs, we may be able to support you at home or in the community.
How to get community support
We can only provide you with community support if you have been assessed as having mental health needs. A qualified mental health professional (CPN, social worker, psychiatrist etc.) must contact us on your behalf. You can also contact us directly at the following addresses:
