Intermediate Care
Intermediate Care is a range of services designed to help you to stay as independent as possible by working with you in your own home or in a residential setting.
How can Intermediate Care help?
We may be able to
- Support you while you are recovering from an illness, allowing you to remain independent in your own home.
- Provide support in your own home or in a residential or nursing home to prevent you having to go into hospital
- Prevent you staying in hospital longer than you need to by helping you to get back to living at home.
- Support your carers, friends or relatives in looking after you when you are not as healthy or mobile as you were.
Who provides these services?
Intermediate Care is a multidisciplinary team operating in partnership with Salford Primary Care Trust, Salford City Council's Community, Health & Social Care Directorate, Age Concern and Independent Providers. It is made up of a wide range of health and social care professionals including carers, doctors, nurses, occupational therapists, physiotherapists, social workers and other specialist staff when required.
How long is this care provided?
Intermediate Care can be short term, from a day or two up to six weeks to help you over a period of crisis. It can also sometimes be longer term, to give you enough time to recover from an illness or an operation.
What services are there?
The Rapid Response team provides short-term care in your home when you are not well and could prevent your admission to hospital. The team works with people who are 18 and over.
The Community Rehabilitation and Supported Discharge team provides treatment and rehabilitation in your home for up to 12 weeks or sometimes longer if necessary. Its aim is to help you achieve as much independence as possible to allow you to remain in your own home rather then move into residential or nursing home care. The team works with people who are 18 and over.
Swinton Hall Intermediate Care Unit is within a nursing home. You could stay there for up to 2 or 3 weeks if you are not very well but do not need to go into hospital, or if you are improving after a stay in hospital but not quite well enough to go back home yet. The unit normally works with people who are 60 or over but sometimes works with younger people.
The Limes Rehabilitation Unit is within a short term residential care home. A stay at the unit can help you to regain your independence and confidence so that you can return home. Stays can last up to a maximum of 6 weeks. The unit normally works with people who are 60 and over, but sometimes works with younger people.
Heartly Green Intermediate Care Unit is within a residential home and can provide nursing or residential care depending on your needs. A stay can last for between 1 and 3 weeks if you are not well but do not need to go into hospital, or if you are improving after a stay in hospital but not feeling well enough to go home yet. If you need more help to regain your independence and confidence, a longer stay of up to 6 weeks may be needed. The unit normally works with people who are 60 or over but sometimes works with younger people.
White Meadows Assessment Unit is in a short term residential care home. People with confusion, memory loss or dementia can be assessed and supported by staff there to find out what can be done to help them to remain independent for as long as possible. The unit normally works with people who are 60 and over, but sometimes works with younger people.
How can I get in touch with these services?
You need to talk to a social worker or a health professional (this could be your doctor, therapist or nurse). Once they have contacted us, we will arrange to see you at home or in the hospital if you are staying there to find out if we will be able to help you. We will ask you questions about the things you find difficult to do. If you agree, we will also ask other people who care for you - your GP, other health and social care professionals or any friends and relatives who help you. This is called an assessment and it will help us to plan an individual programme of care called a care plan with you. This will identify your goals and the support you need to achieve them
How much does the service cost?
Intermediate Care provided in your own home is free, but there may be a charge for care in some residential homes. You will be advised of this at the time of your assessment.
