Integrated care services

Salford City Council and Salford Primary Care Trust are changing how we provide district nursing and social services to adults and older people. Over the next year, we're opening eight Integrated Care Teams. Each team will be based locally and will have both social workers and nurses working together to meet all your health and social care needs.

Your integrated care team will be linked to your GP - so, if your GP is in Walkden, you'll get nursing and social services from the Walkden Integrated Care Team. Find your local social services.

We're opening these teams to make it easier for you to get services, to improve the quality of services and because the Government has told us this is the best way to provide services in ‘Our health, our care, our say’.

The first team - Walkden Integrated Care Team - opened in November 2005 and we asked Manchester Business School to see if there had been improvements. They found that there has been less duplication of work, increased number of assessments, improvements in how quickly we responded to requests and improvements in how quickly people received services.

A user/carer survey, staff survey and views from local GPs also showed improved satisfaction of the new service.

In December 2007, Eccles Integrated Care Team opened, based at Emerson House.

Timetable for opening the new teams

We are planning to open most of the new teams over the next few months - but there might be some delays - you can get the most up to date information about which team to contact from our contact social services.

  • March 2008 - Ordsall Integrated Care Team, based at Ordsall Health Centre
  • April/May 2008 - Swinton Integrated Care Team, based at Blackcroft
  • April/May 2008 - Clarement Integrated Care Team, based at Lanceburn Health Centre
  • May/June 2008 - Broughton Integrated Care Team, based at Sahal Court
  • January 2009 - Little Hulton Integrated Care Team, based at Walkden Clinic

Irlam Integrated Care Team will be based at Chapel Medical Centre - we'll let you know as soon as we have a date.
