The winner is
Advance Decision to Refuse Treatment team and Central Support

Four members of the Mental Capacity Act implementation group led on the development of the Advance Decision to Refuse Treatment card, with the objectives of raising awareness of the Mental Capacity Act and giving residents the opportunity to make advance decisions. The cards were distributed widely across the city and following a media campaign, central support dealt with over 600 email enquiries from across the country, requesting copies of the card.
