Service Performance Indicators (SPIs) These are used to assess performance please see the Service Performance Indicators page for information on how and when to submit them.

National Outcomes Framework forms - All our providers should now be completing these forms online see the client record website for detailed information and guidance.

Client Record Forms - All our providers except sheltered housing, alarm services and the Home Improvement Agency should be filling these in online please see the client record website for detailed information and guidance.

Consultation - this is another way that we assess a service's performance and will include consultation with stakeholders and service users.
