Towards independence - reablement
Reablement is about giving you back as much independence as possible.
There are a range of services, mostly short-term, where we encourage you to do as much as you can for yourself. We will be there to help and support you.
After using reablement services, most people are stronger, more confident and able to do more for themselves.
How does reablement work?
Health or social care staff will talk to you about what you want to be able to do and how you are managing. They will look at different ways that you could do things. These could be really simple solutions; if you struggle to make a hot drink, would a smaller kettle help?
You will have a plan of what you are aiming to achieve and review it each week. If you need some extra equipment to help you, we can show you how to use it, or how to re-arrange your home to make things easier for you.
We will get you to have a go first, before we do things for you.
At the end of the period of reablement (usually up to six weeks), you may not need any more support. If you do, then a social worker will get back in contact with you.
Reablement services are free but you do need to have an assessment showing that you could benefit.
What services are there?
- Intermediate home support is intensive personal home care. It is one of the intermediate care services.
- Intermediate care services are a range of short-term residential and home based services.
- Telecare is a range of sensors that automatically send an alarm to your carer or to Care on Call, if you get into difficulties.
