Here you will find the following documents that you may require when booking your activities, or before you attend sessions at Salford Watersports Centre.

All the forms can be downloaded at the bottom of this page.

Personal infomation and course booking form

To book onto your course please return the booking form with full payment for your course. For group bookings do not use this form, please contact the centre for group bookings. This form is also used as the personal infomation form that all participants on public sessions have to complete each year. We must have a completed signed and up to date copy of this form before any person can take part in activities.

This is a list of what you will need to bring with you on your visit to the watersports centre.

Group information pack

This pack contains all the forms you will need to give out to your group before attending. Each member of the group must complete a personal information form which is included in this pack before they can commence activities. Also included is a map of how to find us, and a group activity register so that you can split your whole group into relevant activity groups before you arrive.

Personal information form

Any person coming as an individual (for example not with a group) must complete a copy of this form before their first visit of the year.

Open water swim registration form (will be available soon)

All open water swimmers must complete and return a copy of this form before attending the open water swim. Registration will not take place on a Monday or Thursday evening so pre-registration is essential. Please make sure you register with a 2011 form, as forms from previous years will not be accepted.

Group leader information and behaviour policy

We want to ensure that your sessions run as smoothly as possible and to ensure that everyone gets as much enjoyment from the activities as possible. To do this, we have introduced a 'group leader' information sheet and a 'behaviour policy'. Please ensure that the leader that brings the group is familiar with this, and that the participants are aware of what is expected from them. These are available to download below.
