Salford City Council is committed to supporting residents facing financial hardship and poverty during end-of-life care.

Health care professionals and partners who issue SR1 forms can help patients access vital assistance by completing the below online form.

Help and support SR1 form

Consent from the patient must be obtained before submission.

Available support includes:

Council Tax Support

  • Recovery or bailiff action can be paused for patients in arrears. Exemptions or discounts may also be explored.

Discretionary Financial Help

Means-tested aid such as:

  • Free school meals
  • Shopping vouchers
  • White goods
  • Help with gas and electricity bills
  • Discretionary Housing Payment
