How we use your information

The council will use your personal information for the purpose of delivering the service as requested by you by completing this form. Application details

Please only use this form to notify us about changes in your circumstances that will affect ongoing payments in respect of the Local Restriction Support Grant. (opens in a new window) or the Additional Restrictions Grant. (opens in a new window). This could be:
 

  • your business has stopped trading
  • you are no longer the owner of the business
  • your business has gone into administration or received a striking off notice 
  • or any other reasons that you should no longer receive a grant payment
     

You can also use this form to tell us if your bank account or payee details have changed. There is the facility at the end of the form to upload relevant evidence such as copy bank statements. For example, if you are changing bank accounts, please upload a copy of both the new account details and a recent statement from the previous bank account.

Do not use this form if you want to tell us about general changes to your Business Rates billing account that would not affect ongoing Support Grant payments, or
