Prevent: Referral form and guidance

This referral pathway is currently being updated. Please find underneath the new National Prevent Referral Form.

This referral form is only to be completed if the person you are worried about is at risk of radicalisation or terrorism. 

This is not a safeguarding referral and does not go to The Bridge. If you have safeguarding concerns about a young person or adult you must fill in a worried about an adult/ child safeguarding referral.
