Request a replacement personal licence

To request a replacement personal licence due to loss or damage, or due to a change of name or address, you will need to:

• Pay the £10.50 fee now by debit or credit card
• Upload evidence of a name or address change. A utility bill can be used to confirm an address change. A marriage certificate or Decree Nisi should be used for a name change
• Provide your personal licence number. If you don’t know this you can find it on the public licensing register

If you are requesting a change of name or address, you must return your paper licence and badge to:
