Primary Care Networks

Primary Care Networks (PCNs) are groups of GP practices working together with community, mental health, social care, pharmacy, hospital and voluntary services in their local area.

PCNs build on existing primary care services and enable greater provision of proactive, personalised, coordinated and more integrated health and social care for people close to home. 

Salford has five PCNs, each with services located in different areas of the city. Most patients live near their GP practice, but they may be more widely spread around the city.

The five PCNs in Salford are:

  • Broughton Health Alliance PCN
  • Eccles and Irlam PCN
  • Salford South East PCN (formerly Ordsall and Claremont PCN)
  • Swinton PCN
  • Walkden and Little Hulton PCN
